Dear Parent or Guardian:
State Law requires each student enrolled in a Georgia Public School to have a current immunization record on Form 3231 provided by the Georgia Department of Human Resources.
YOUR STUDENT NEEDS A CURRENT IMMUNIZATION RECORD – FORM 3231 – AT SCHOOL IN THEIR PERMANENT FILE. THIS FORM CAN BE OBTAINED AT THE PHYSICIAN’S OFFICE OR THE TROUP COUNTY HEALTH DEPARTMENT ON DALLIS STREET-706-845-4085.
